New Patients

Your First Visit

Please arrive 15 minutes early to complete your paperwork. The New Patient Packet includes general information, medical history forms, and HIPAA privacy information. For your convenience, for a link to the packet click here.

Please bring:

• Your prescription (if required)

• Your insurance card

• A photo ID

• Any relevant test results

When you arrive, our staff will greet you and help you get settled. After your paperwork is completed, your therapist will bring you to a private treatment area.

Your therapist will review your history, discuss your symptoms and goals, and perform a thorough evaluation. Based on this assessment, we will create a personalized treatment plan to help you recover and reach your goals.

Before leaving, our scheduling team will help arrange your follow-up appointments.
